Enrolling a New Dependent
New Born Child
A new born child is eligible for coverage on the date of his or her birth. The fastest way to add them to your coverage is to submit the Dependent Enrollment Form and include the Hospital Birth Announcement. The child's full name, date of birth and Member's name must be stated on the Announcement. Make sure to send Birth Certificate once received.
Stepchild or Adoption
If you would like to enroll a stepchild from a marriage or have adopted a child, please submit the Dependent Enrollment Form. You will need to make sure you have the child's Birth Certificate and Social Security Card to upload during the enrollment process.
Marriage
If you would to enroll a spouse, please submit the Dependent Enrollment Form. You will need to make sure you have the spouse's Birth Certificate, Social Security Card, and the Marriage Certificate.
Please include copies of all Health Insurance Cards any Eligible Family Member is enrolled in besides MARCC Health Coverage. You will also need to indicate the effective date and which benefits are covered such as; Medical, Dental, Vision or Prescription.
